Uveďte ulicu, ak sa obec člení na ulice a súpisné/orientačné číslo bydliska žiadateľa. Ak sa obec nečlení na ulice, uveďte len súpisné číslo.

Fyzická osoba (podnikateľ) uvedie obchodné meno zdravotnej poisťovne, v ktorej je prihlásená na povinné zdravotné poistenie / Natural person (entrepreneur) shall provide a business name of the health insurance company, where he/she is registered for mandatory health insurance

Podľa § 57 ods.1 písm. g) zákona č. 455/1991 Zb. o živnostenskom podnikaní (živnostenský zákon) v znení neskorších predpisov, oznamujem ukončenie podnikania / Pursuant to Article 57 par. 1 letter g) of Act No. 455/1991 Coll. on Trade Licensing (Trade Licensing Act) as amended, I notify termination of business:

Ukončenie vo všetkých predmetoch podnikania / Termination in all lines of business

Dátum ukončenia podnikania je dňom zániku živnostenského oprávnenia (§ 57 ods. 1 písm. g) živnostenského zákona) - podnikateľskú činnosť už nie je možné vykonávať.

/ If in the notification pursuant to paragraph 1 letter g) no later date is indicated, then the trade  authorisation shall expire as of the next day of the day when the notice of termination of trade is served upon appropriate Trades Licensing Office.

Ukončenie vo všetkých predmetoch podnikania uvedených na vybraných osvedčeniach o živnostenskom oprávnení / Termination Of all lines in business indicated on the Certificate of Trade Authorisation

Dátum ukončenia podnikania je dňom zániku živnostenského oprávnenia (§ 57 ods. 1 písm. g) živnostenského zákona) - podnikateľskú činnosť už nie je možné vykonávať.

Oznámenie o ukončení podnikania možno vziať späť najneskôr v deň , ktorý predchádza dňu zániku živnostenského oprávnenia. (§ 57 ods.3 živnostenského zákona) / Notification of termination of trade may be retracted at the latest on the day which precedes the day of expiration of the trade authorisation (Article 57 par. 3 of Trade Licensing Act).
