Čestne vyhlasujem že som osobou uvedenou v § 9a ods. 6 písm. a) až g) zák. č. 138/1991 Zb. o majetku obcí v znení neskorších predpisov, ktorá je:

b) poslancom obecného zastupiteľstva
c) štatutárnym orgánom alebo členom štatutárneho orgánu právnickej osoby zriadenej alebo založenej obcou
d) prednostom obecného úradu
e) zamestnancom obce
f) hlavným kontrolórom obce
g) blízkou osobou osôb uvedených v písmenách a) až f)
Toto čestné vyhlásenie som vykonal (a) dobrovoľne a s plnou zodpovednosťou pre potreby obce za účelom predloženia mojej žiadosti vlastníctva z majetku mesta alebo prenechanie majetku do nájmu.
Zároveň vyhlasujem, že tieto údaje sú pravdivé a úplne Som si vedomý/á právnych následkov nepravdivého čestného vyhlásenia v zmysle zákona č. 71/1967 Zb. o správnom konaní v znení neskorších predpisov.
*Vysvetlivka - § 116 Obč. zákonníka. Blízkou osobou je príbuzný v priamom rade, súrodenec a manžel, iné osoby v pomere rodinnom alebo obdobnom sa pokladajú za osoby sebe navzájom blízke, ak by ujmu, ktorú utrpela jedna z nich, druhá dôvodne pociťovala ako vlastnú ujmu.
